FormBlends Review

FormBlends publishes the most completely documented price on this roster. Four plan lengths for each molecule, the one-time price and the subscriber renewal side by side, the plan total beside every per-month figure, a stated introductory month, and a date on which the whole table was last checked against the checkout code. It is also a seller that publishes provider-comparison editorial, so a figure found elsewhere on the site may belong to someone else.

$99a twelve-month prepayment: "12 months $1,188 · $99 per month … billed per 12-month period"; the page prints the total beside the per-month figureVerified September 2026source

A price table that dates itself

The pricing page states when it was last verified against the checkout code, to the day. Nothing else on this roster does that, and it is the single most useful sentence on any pricing page we read this week.

Each molecule gets four plan lengths — monthly, three months, six months and twelve — with the one-time price and the subscriber renewal in adjacent columns and the plan total printed. The discount is arithmetic a reader can check rather than a claim.

It also states, in words, that the price does not change with the dose.

An introductory month, and an honest note about it

The first month of any plan is charged at a lower introductory rate, stated separately for each molecule. The page then adds something unusual: on the twelve-month plan the introductory figure changes nothing, because that plan is already at the same rate.

That is a seller correcting the impression its own promotion creates. We have recorded both introductory rates as offers that revert, and the reversion is roughly double on the shorter plans.

It also writes about its rivals

This site publishes cost articles comparing named and unnamed providers, with tables of their prices. Those are not its prices, and a reader — or a price extractor — pointed at one of those pages would harvest a figure belonging to another company.

Everything recorded here comes off its own pricing page. It names its operating company, its state of formation and its street address consistently across the footer, the privacy notice and the terms; it reviews its own clinical content under a team name rather than a person’s, and it names no compounding pharmacy.

Good fit if

  • Readers who want a published price they can check and a date it was checked.
  • Buyers comparing plan lengths with the totals shown.
  • People who want the introductory month and the reversion stated together.

Look elsewhere if

  • Anyone who wants the headline figure to be the ongoing one.
  • Readers who want a named compounding pharmacy or clinician.
  • Buyers who need a service-area list.
  • People who might mistake its editorial tables for its own prices.

How this review was verified

  • Re-read the pricing page, the privacy policy and four editorial pages on 7 September 2026, comparing requested to effective URLs with a nonsense path against the host in the same run, which returned a 404.
  • Recorded only figures from the seller’s own pricing page, and excluded every figure from its comparison articles.
  • Recorded the twelve-month plans as prepayments rather than monthly rates, because the page states the billing period.
  • Read the operating company, its state of formation and its address in three places and confirmed they agree.
  • Screened both spellings of the operating company against FDA’s live warning-letter datatable with both controls green: no letter found.
